If you are keen to join the SCDAS, just follow the instructions below to join us:
1.Download the Membership Registration Form and filling it in.
2.Submit the corresponding membership fees via:
(i) Internet Banking to “Special Care Dentistry Association of Singapore” OCBC account 5194-5074-6001. Do take note of the trans/ref no. and notify us of your transaction in your application form.
(ii) Cheque, payable to “Special Care Association of Singapore“, double-crossed, and sent via mail to Block 338, Ang Mo Kio Avenue 1, #01-1673, Singapore 560338. Kindly notify us of your cheque payment together in your application form.
(iii) Cash, and this can only be submitted in person to the Treasurer/Secretary during events, activities or meeting. Hence, cash payment is not encouraged.
3.Send the duly completed Member Registration Form:
(a) in softcopy (.pdf or docx format) to specialcaredentistrysingapore@gmail.com
(b) in hardcopy to Block 338, Ang Mo Kio Avenue 1, #01-1673, Singapore 560338
4.Our Registration Team will process your application and come back to you in 7-10 working day!
